Believe this is a misunderstanding around the "first-time" terminology. The variants you're describing unlock if she's visibly pregnant the first time you play that scene in a playthrough—not the first time you play that scene while pregnant. Only one of the three can be unlocked in any given play-through. This is a known point of confusion and steps have been taken to help reduce player confusion in that regard for upcoming builds. |
